Claim 130 miles account I worked beyond the limits of my assignment <list

from <list time> to <list time>. I worked beyond the limits because

Claim 130 miles account required to perform service that is not part of my

regular assignment to <explain what work you performed off your
assignment>. Per Article 6, Section 6 ( c ). Please refer to my working

100 MILES YARD DAY (mark Penalty)
Claim 100 mile yard day, account I switched cars number <list first car

employed between <list time> and <list time>. Note this was not part
of the three (3) pick-ups allowed at initial terminal, three (3) set outs

of loss of (put one of the following) A. Brother, B. Sister, C. Mother, D.
Father, E. Child, F. Spouse, G. Spouse. s Parent, H. Half-Brother, or I.
Half-Sister. Per Article 12 of the 1995 Agreement. Obituary notice was faxed to
8-997-2381 on <list date> at <list time>. (Note: when you fax the
obituary notice please put your name and social security number and claim number
on the fax.)
PHYSICAL EXAM, SAFETY MEETING, INVESTIGATION (mark
Company Business)
