Claim 65 miles (4 hours) account I was rested, available and ahead of Engineer <list Engineers Name> on <Date> when Engineer <list Engineers Name> was used around me on the <list train ID> on duty at <list time> on <list date>. I was called at <list time> on <list date> behind Engineer <list Engineers Name>, for the <list train ID>. Per Article 30 Section 5 (a). YARD RUNAROUND (mark Penalty)

Claim 65 miles (4 hours) account I was called on duty for the <list train ID> at <list time> and I departed at <list time>. Engineer <list Engineers Name> was called on duty at <list time> and departed ahead of me at <time>. Both of us were in the same yard, on the same pool, power was on both trains, going in the same direction, called off the same board and I was on my train and ready to go. DIFFERENCE OF EARNINGS (mark Lost Earnings)

I was used in emergency off my regular assignment and claim the difference of earnings of assignment <list assignment number> on <list date> on duty at <list time> which is the job that I was used in emergency on, and claim the round trip earnings of my regular assignment I would have worked, assignment <list assignment number> on duty at <list time> on <list date> with Engineer <list Engineers Name>. EARNINGS (mark Lost Earnings)

Claim the full earnings of Engineer <list Engineers Name>, on assignment <list assignment number>, on duty at <list time> on <list date> account (use one of the following) A. I was used off my regular assignment on the previous calendar day, Article 12 Section 1 (d), B. I was used off my regular assignment on Company business, Article 12 Section 1 (d). 130 MILES CALL AND RELEASE (mark Penalty)

Claim 130 miles call and release, I was called at <list time> for <list time> on duty and was released at <list time>. (use one of the following) A. I was held over 4 ours on duty before I was released, B. This is because I performed service, <explain the service performed>, Article 19, Section 2 (a). I retain my position on the board per Article 19, Section 2 (a) of the 1995 Engineers Agreement. Please refer to my work history.

65 MILES CALL AND RELEASE (mark Penalty)

Claim 65 miles (4 hours) call and release, I was called at <list time> for <list time> on duty for assignment number <list assignment number> and was released at <list time>. (Put one of the following down A. Less than 4 hours on duty and I did not perform service, B. Release before leaving home, C. On duty time was changed before I left home and I was not notified within 15 minutes of the original call, D. On duty time was changed by more than 45 minutes before I left home.) Per Article 19, Section 2 of the 1995 Engineer Agreement which supersedes Article 30 Section 1 (a), (b) and (f) of the Engineer Agreement. Please refer to my work history.

130 MILES WORKING BEYOND THE LIMITS OF ASSIGNMENT (mark Penalty)

Claim 130 miles account I worked beyond the limits of my assignment <list assignment number>. The limits are MP <list MP limits> to MP <list MP limits> and I worked to MP <list MP you worked to> beyond the limits from <list time> to <list time>. I worked beyond the limits because <explain the reason why you worked beyond your assignment> and was instructed by <list the company officer> to do this. This claim is minus overtime paid on working timeslip <list timeslip number> dated <list date>, up to time consumed for working off assignment. Per Article 19 section 3.

130 MILES OFF ASSIGNMENT (mark Penalty)

Claim 130 miles account required to perform service that is not part of my regular assignment. On <list date>, at <list time>, I was instructed by <list company officer that instructed you to do this> to go off my regular assignment to <explain what work you performed off your assignment>. Per Article 6, Section 6 ( c ). Please refer to my working timeslip number <list timeslip number>, dated <list date> and to the job bulletin for my assigned job.

100 MILES YARD DAY (mark Penalty)

Claim 100 mile yard day, account I switched cars number <list first car number> through number <list last car number> on track(s) number(ed) <list track(s) number(s) at <list Circ 7>, where yard engines are employed between <list time> and <list time>. Note this was not part of the three (3) pick-ups allowed at initial terminal, three (3) set outs allowed at final terminal nor because of a bad order car. Per Article 11, Section 4 (a). REGULAR ENGINEERS NOT USED ON ASSIGNMENT (mark Penalty)

Claim <list miles> Bulletin miles, minimum 100 miles at last rate of pay. I was not used on my regular assignment through no fault of my own. (put one of the following down A. Job was blanked, Article 12, Section 1 (b); B. Job was set back to next calendar day. Article 12 Section 1 (b).) Scheduled on duty time was <list time>, on run number <list run number>.

CALLED AHEAD OF ASSIGNMENT (mark Penalty)

Claim 130 miles account called at <list time> ahead of bulletined on duty time of <list time>, for assignment number <list assignment number>, per Article 6, Section 6 (b). Please refer to my working timeslip number <list timeslip number>, dated <list date> and to the job bulletin for my assigned job.

JURY DUTY (mark Other)

Claim time lost for jury duty on the following dates <list dates>. Minimum of basic day for each day summoned for Jury Duty. Per Article 15 of the 1995 Agreement. Copy of Jury Summons and time card from court were faxed to 8-997-2381 on <list date> at <list time>.

BEREAVEMNET LEAVE (mark Other)

Claim 3 days bereavement leave for lost time on the following dates account of loss of (put one of the following) A. Brother, B. Sister, C. Mother, D. Father, E. Child, F. Spouse, G. Spouse. s Parent, H. Half-Brother, or I. Half-Sister. Per Article 12 of the 1995 Agreement. Obituary notice was faxed to 8-997-2381 on <list date> at <list time>. (Note: when you fax the obituary notice please put your name and social security number and claim number on the fax.)

PHYSICAL EXAM, SAFETY MEETING, INVESTIGATION (mark Company Business)

Claim full lost earnings in the amount I would have earned, with a minimum of a basic day for attending <Put physical exam, safety meeting or investigation>, at the request of the carrier on <list date>. Per Article 27, Section 1 (a), (b), ( c).
